Description of Program and Relief

HealthWell Foundation

HeathWell Foundation is providing grants of up to $250 to assist individuals with ancillary costs associated with the COVID-19 outbreak. These costs may include, but are not limited to, food or medicine deliveries, diagnostic testing, and telehealth services.

Eligibility Requirements/Restrictions

To qualify for assistance, an applicant’s income must fall below 500% of applicable Federal Poverty income level guidelines and meet at least one of the following requirements:

  • A member of applicant’s household has been diagnosed with COVID-19;

  • A member of applicant’s household is part of a population that is at a high risk for contracting, and suffering complications from, COVID-19;

  • Applicant resides in a geographic area of the United States or one of its territories where COVID-19 is prevalent; or

  • A physician or government official recommended that a member of applicant’s household self-quarantine due to possible exposure to COVID-19.
