Description of Program and Relief

Remote Patient Monitoring Program for Quarantining Individuals

In light of COVID-19, Illinois has launched a remote patient monitoring program. Under the program, Pandemic Health Workers will use technology to connect with people experiencing COVID-19 symptoms who need to quarantine. The Pandemic Health Workers will virtually visit recipients every day, and deliver wellness kits with essential tools to monitor their health, such as thermometers, pulse oximeters, blood pressure cuffs, and alcohol wipes.

Eligibility Requirements/Restrictions

Individuals must call the COVID-19 hotlines below for a final determination of eligibility.

Individuals may be eligible if they are showing symptoms of COVID-19, or are at high risk for contracting the virus, but do not require emergency or inpatient care.
